Outpatient Care

Community Hospice’s total team care recognizes the needs of the patient, the family, and friends because a terminal illness creates various types of stress and responsibilities. An interdisciplinary team of caregivers will work with you and your family to offer as much support as you request including the following range of services:


  • Physical: bathing, dressing, meal preparation, exercise, and housekeeping
  • Emotional: providing understanding and counseling to manage emotions
  • Social: ensuring that the patient has access to social activity as his/her condition comfortably warrants, to remain connected to others
  • Spiritual: chaplains help the patient, his/her family, and friends, and manage the spiritual anxieties that accompany a terminal illness
Nursing services include a nurse making home visits to assess the patient’s condition, teach caregivers how to properly care for the patient and particularly how to manage symptoms such as pain, nausea, constipation and vomiting. The nurse will communicate regularly with the patient’s primary physician and arrange for special equipment and supplies as needed.
